Terms and Conditions
1. By submitting this form, I authorize FDA Agent Pro and their associates to submit the FEI number on behalf of our firm
2. I agree that all the above information is Truthful and Accurate
3. I am aware that any fee paid is a service charge of the FDA fee and non-refundable unless FDA Agent Pro fails to provide the requested services.
4. If our firm is unable to provide the requested information in a timely manner, it will not be considered FDA Agent Pro failure.
5. I understand that the FEI assignment timeline is not under FDA Agent Pro's control, and the timeline to obtain the FEI number depends on the FDA's response time.
6. You agree to assume all risk, release FDA Agent Pro LLC officers, directors, employees, affiliates, and agents from any and all liability in connection with this product, waive all claims, and will hold harmless and indemnify FDA Agent Pro LLC and its officers, directors, employees, affiliates, and agents from any and all claims in connection with this application.
7. It may take up to 2 weeks to receive a FEI from FDA.
